1) Please provide a PV for scale, clean and polish, I do not see it on the website.

2) Please provide an email from the dentist performing the treatments, not the clinic, 

We need to have an email confirmation from each GDC/IDC member taking part in this deal to state the following:

       a: Full name

       b: GDC/IDC number

       c: Job title

       d: Day and working times at location.

3) FP say: ‘Rescheduling/cancellation policy:​ 48 hour​s’, however, FP also says ‘Ensure 72 hour clock is mentioned to give people time to change their minds with full refund’. This is contradictory information. Please remove one of the sentences from the fine print and get a new esign.
